FDA Approves Bristol Myers Squibb’s Zenbexus for Multiple Myeloma

The U.S. Food and Drug Administration has granted accelerated approval to Bristol Myers Squibb’s Zenbexus (iberdomide), introducing a new treatment option for adults with multiple myeloma whose disease has returned or stopped responding to treatment. The approval, announced on August 13, covers Zenbexus in combination with daratumumab and hyaluronidase-fihj and dexamethasone for patients who have previously received at least one line of therapy including a proteasome inhibitor and an immunomodulatory agent.

Zenbexus for Multiple Myeloma

Multiple myeloma is a cancer of plasma cells, a type of white blood cell that can accumulate in the bone marrow and interfere with normal blood-cell production. While treatment options have expanded substantially, relapse remains common and patients may eventually become resistant to existing therapies. Around 36,000 new cases of multiple myeloma are expected to be diagnosed in the United States in 2026.


Zenbexus is notable because it is the first FDA-approved therapy from a new class known as cereblon E3 ligase modulators, or CELMoDs. These medicines are designed to bind to the cereblon protein and trigger the degradation of proteins that help myeloma cells survive. The approach builds on earlier immunomodulatory therapies while introducing a new mechanism for targeting the disease.


The FDA’s decision was supported by results from the Phase 3 EXCALIBER-RRMM trial. In the primary efficacy analysis, 41% of patients receiving the iberdomide combination achieved a minimal residual disease-negative complete response, compared with 21% of patients receiving daratumumab, bortezomib and dexamethasone. Minimal residual disease testing can detect very small numbers of cancer cells that may remain after treatment, offering a way to assess the depth of a patient’s response.


The study is continuing to evaluate progression-free survival, which measures how long patients remain alive without their disease worsening. Because Zenbexus received accelerated approval, continued authorization will depend on confirmation of clinical benefit through further analysis or confirmatory trials.


Safety remains an important consideration. Zenbexus carries boxed warnings for embryo-fetal toxicity and serious venous and arterial thromboembolism. The FDA also lists risks including neutropenia and infections, and the medicine will be available through a restricted distribution program.


The approval gives clinicians another option for patients facing relapsed or refractory disease while also marking the arrival of a new therapeutic class in multiple myeloma. As longer-term data emerge, attention will now turn to whether the early depth-of-response results translate into durable improvements in disease control.
